While there was a management structure in place and staff understood their roles, some risk assessments and monitoring charts were not consistently updated or completed accurately, leading to potential risks. Internal audits had not always identified these shortfalls, though quality monitoring processes were under review. Despite these areas for improvement in leadership and oversight, the management team was described as supportive, and staff felt confident reporting concerns, contributing to a generally positive workplace culture.
