What inspectors found

This inspection report for Carlile Lodge by the Care Quality Commission (CQC) focuses on the care provided to ten individuals with learning disabilities and/or autistic spectrum disorder, who may also exhibit challenging behaviours. The service operates as a care home where residents live in their own flats while sharing communal spaces. This report details the findings of an unannounced inspection conducted on February 26, 2018, assessing the service across five key areas: safety, effectiveness, caring, responsiveness, and leadership. Overall, the inspection found the service to be consistently well-regarded, with strengths identified across all domains.

On safety, residents are actively involved in understanding their vulnerabilities and planning how to stay safe, with staff support and referrals to specialist teams when needed. The service ensures that risks are assessed collaboratively with residents, and environmental safety is regularly monitored. Robust recruitment processes are in place, and residents are involved in the hiring of new staff. Medicine management is also a focus, with residents encouraged to manage their own medications where possible and supported through assessments and clear guidance for staff. Incidents and accidents are thoroughly reviewed and discussed by staff to promote continuous learning and prevent recurrence.

In terms of how effective the care is, residents have significant choice and control over their daily lives and support, with staff intervening only when necessary. Staff are well-trained and supported, with ongoing development and effective communication systems in place to ensure resident needs are consistently met. Residents are fully involved in choosing their meals and managing their health, with support to understand conditions and attend appointments. The environment is designed to meet individual needs, including necessary adaptations for comfort and safety.

Regarding caring practices, residents consistently described staff as kind, compassionate, and knowledgeable about their individual needs and histories. Staff are attentive to residents' emotional well-being, offering support and understanding during difficult times. Communication is tailored to individual preferences, with information provided in accessible formats. Residents are supported to maintain relationships with family and friends, and staff help them navigate the complexities of personal relationships safely.

In terms of responsiveness, the service ensures residents are actively engaged in a variety of enjoyable activities and supported to achieve personal goals, such as employment or holidays. Care plans are developed collaboratively with residents, clearly outlining support needs and professional guidance. The service has a clear complaints policy, and feedback is actively sought and acted upon. Importantly, residents are sensitively supported in planning for end-of-life care, respecting their wishes and understanding.

Regarding leadership, the registered manager and provider are seen as available, supportive, and focused on continuous improvement. There is a clear vision for the service that prioritizes resident needs and preferences. Staff are empowered to contribute ideas, and open communication is encouraged. Regular audits are conducted to monitor quality, and feedback from residents, families, and professionals is actively sought and incorporated into action plans. The service demonstrates strong collaboration with external professionals, ensuring residents receive comprehensive support.
