This report summarizes findings from an inspection of Woodford House, a care home providing personal and nursing care. While some residents and their families offered positive comments, the inspection identified significant and widespread issues across most areas of the service, impacting residents' dignity, well-being, and the overall management of the home. Regarding safety, the home did not consistently ensure residents were protected from harm. Risks to residents were not always fully assessed or mitigated, with insufficient guidance for staff on how to manage complex health conditions, mental health issues, and specific care needs like dialysis. There was also a lack of robust procedures for learning from incidents, and recruitment processes were not always safe, leaving potential gaps in staff history checks. While some areas like infection control and medication management were found to be satisfactory, broader safety concerns were noted.
In terms of how effective the care is, residents' support did not always achieve good outcomes or was inconsistent. A key concern was a lack of understanding and application of the Mental Capacity Act by staff, leading to inappropriate restrictions and inadequate documentation of capacity assessments and best interest meetings. Furthermore, staff lacked specific training to support residents with learning disabilities and autism, impacting their ability to communicate and de-escalate distress.
Concerning caring practices, residents were not always treated with kindness, dignity, and compassion. This manifested in residents being referred to by numbers rather than names, and their personal preferences for dressing not being respected, which severely impacted their sense of self. Some parts of the home also suffered from noticeable odours, indicating a lack of attention to the environment.
When considering how responsive the service is, families found contacting the home challenging and were not always kept updated on their loved ones' care. While end-of-life care plans were in place, they lacked necessary detail. Complaints were also not effectively documented or responded to, and there was a lack of accessible information and communication for residents.
Finally, looking at how well-led the service is, there was a significant lack of oversight and effective leadership. Audits were completed but lacked deadlines and clear support for implementing improvements. Opportunities for learning from incidents were missed, and there were concerns about the documentation of accidents. Despite a recent provider change, improvements were slow, and a compliance audit was delayed. The provider has been required to submit monthly reports to demonstrate progress.
