On how well-led the service is, while there are quality assurance processes, they did not always identify areas for improvement, including potential infection risks and environmental issues. Governance systems need to be strengthened to consistently support safe care delivery. Policies and systems were not always fully supportive of best interest decisions, with limited evidence for how these were made and capacity assessments not always consistently recorded.
