What inspectors found

Bonhomie Sarisbury Green provides residential care for up to 14 individuals aged 18 to 65 with substance misuse or mental health conditions. This report from the Care Quality Commission highlights areas where the service excels and where improvements are needed to ensure the highest standard of care.

On safety, the inspection noted concerns regarding the management of medicines, with some records found to be incomplete or made in error, and insufficient detail for administering 'as needed' medications. The premises also required attention, with maintenance issues like scuffed paintwork and unsealed flooring edges impacting cleanliness and the ability to effectively sanitise. While safety checks were in place and the manager was responsive to immediate concerns, including a critical issue with the availability of hand sanitiser, further work is needed to ensure all safety protocols are consistently applied and documented.

In terms of how effective the care is, the report found that care plans were sometimes too lengthy and lacked clarity, making it difficult for staff to fully implement the planned support. While staff received training and were recruited safely, there was a need for more specific training in the medical conditions residents live with, and mental health training had not been updated recently. Concerns were also raised about a swallowing assessment not being followed and a lack of oral health assessments in practice, though the manager agreed to address these.

The service was found to be caring, with staff described as kind, knowledgeable about residents' preferences, and supportive of their independence and choices. Staff understood the importance of supporting individuals in the least restrictive way possible, and advocates were available. However, there were instances where it was not assured that people were always treated with respect or that interactions during crises were always therapeutic.

Responsiveness was a strength, with the service generally supporting people to have maximum choice and control over their lives. Advocates were available, and consent forms were in place. While there wasn't an overarching activities program, individual plans catered to residents' interests. The service also maintained contact with friends and relatives, especially during the pandemic.

In terms of being well-led, audits were conducted but did not always identify areas for improvement. The provider did, however, actively seek feedback through questionnaires and develop action plans. Partnerships with health and social care professionals were reported as good. However, the report highlighted that the service needed to address specific regulatory requirements, such as ensuring learning disability and autism are appropriately registered if supporting individuals with these conditions, and fully embedding the principles of Right Support, Right Care, Right Culture. The service also needs to ensure that quality assurance processes consistently identify and drive necessary improvements.
