This report summarises the findings of a recent inspection at Derby House, a care home providing personal and nursing care for up to eight individuals with learning disabilities, including autism. The inspection focused on how well the service is functioning, with particular attention to safety, effectiveness, caring practices, responsiveness, and leadership. The overall aim of the inspection was to ensure residents are receiving high-quality, person-centred support that promotes independence and a fulfilling life, in line with best practice guidance for supporting individuals with learning disabilities and autism.
Regarding safety, residents are well-protected from avoidable harm. Staff are vigilant in reporting concerns and safeguarding procedures are clearly understood. Risks to residents are thoroughly assessed and managed, with measures in place to prevent recurrence. While medication administration is generally robust, the report notes that 'as required' medicine protocols were not consistently present and this was discussed with the manager for immediate action. The home is maintained to a high standard of cleanliness and hygiene.
In terms of how effective the care is, residents receive support that consistently leads to good outcomes and a high quality of life. Assessments are person-centred, and staff are appropriately trained and skilled, including specific training on autism and epilepsy. Residents are supported to maintain a balanced diet, with choices and involvement in food preparation encouraged. Staff work collaboratively with external agencies, ensuring seamless and effective care, especially for those with complex needs. The adapted environment, including personalised rooms and accessible outdoor spaces, further supports residents' needs and independence.
When it comes to caring, staff are consistently described as kind, compassionate, and attentive, treating residents with dignity and respect. Residents feel comfortable and happy in their home, with staff actively encouraging them to make their own decisions about their daily lives. There's a clear focus on understanding individual preferences and interests, fostering easy-going and respectful interactions. Professionals and relatives alike commend the staff for their dedication and the high regard in which residents are held.
The service is exceptionally responsive to the needs of its residents. Individuals are actively involved in their care planning, taking a key role in the local community and accessing opportunities for education and work. Staff make significant efforts to provide varied activities that reflect residents' interests, from on-site activities to national projects and even holidays abroad. Importantly, when residents' needs change, staff are proactive in upskilling themselves to ensure continuity of care and enable residents to remain in their home.
Finally, in terms of how well-led the service is, there is strong leadership provided by the registered manager. Staff report a positive team ethos and feel well-supported by management and colleagues. Quality monitoring systems are effective in ensuring high standards of care. While residents and their relatives have opportunities to share their views, the report indicates that these are actively listened to and acted upon, contributing to the overall positive culture of the home.
