Downs House is a residential care home that supports up to 49 people, including those living with dementia. Regarding safety, the inspection found that the management of medicines was not always safe, and risks associated with the premises, such as fire safety and secure storage of hazardous items, were not always adequately managed. While staff recruitment processes were robust and infection control measures were reassuring, there were specific concerns about fire doors, evacuation routes, and the provision of window restrictors.
In terms of effectiveness, the service was not consistently ensuring that care and treatment achieved good outcomes for residents. A key area identified was the adherence to the Mental Capacity Act 2005, particularly concerning decisions about bed rails and other aspects of daily life where residents' capacity needs to be properly assessed. Furthermore, staff training records revealed significant gaps in essential areas, meaning some staff might not have the full knowledge required for their roles.
When it comes to the caring aspect, staff were observed to be kind and supportive, treating residents with dignity and respect. Feedback from relatives indicated a feeling that their loved ones were safe and well-managed. The home environment itself was noted as being spacious, light, and suitable for residents' needs, with communal areas like a coffee bar offering social opportunities.
For responsiveness, the home demonstrated some good practices, such as ensuring residents could access necessary healthcare services and that the physical environment was adapted to meet their needs, including accessible gardens. However, the effectiveness of the service in supporting residents with dietary needs and managing risks like malnutrition was found to be inconsistent, with a need for clearer plans and more frequent reviews.
Concerningly, the leadership and governance of the home required improvement. Audits were not always effective in identifying and driving necessary improvements, and some records related to safety checks were incomplete. While the registered manager has been responsive to feedback and is putting plans in place, the overall leadership and management systems needed strengthening to consistently assure high-quality, person-centred care.
