Inspected 7 December 2022 · report published 27 January 2023What inspectors found
Hilltop Court Nursing Home underwent a CQC inspection in December 2022, revealing areas that require ongoing attention to ensure the best possible care for residents. The inspection found that while efforts have been made to address previous concerns, particularly regarding medication management and safety improvements to equipment, there are still inconsistencies in how well residents' needs and preferences are fully met. The report highlights that not all residents consistently receive the maximum choice and control over their lives, and staff do not always support individuals in the least restrictive way. This suggests a need for greater embedding of established policies and systems to ensure consistent best practice.
In terms of safety, while medication administration and storage have improved, there are inconsistencies in documentation related to prescribed thickening products and creams, with some products found to be out of date. While risk assessments and care plans are in place, some lack sufficient detail to guide staff effectively, especially when residents experience distress. Recruitment processes are robust, and staffing levels appear adequate, with residents feeling safe. Infection control measures are well-assured.
Regarding effectiveness, residents' nutritional and health needs are generally met, with positive feedback on food quality, although the variety of daily options could be enhanced. While staff collaborate effectively with external agencies, a recent focus on oral health needs to be re-established. Environmental improvements are ongoing, but the placement of desks in lounges makes them feel less homely, and some bedrooms lack personalization. Issues with up-to-date Mental Capacity Act assessments and Deprivation of Liberty Safeguards authorisations were identified and promptly addressed.
In the area of responsiveness, staff are generally noted to know residents well, understanding their individual preferences. However, the documentation of these needs and preferences within care plans requires further improvement, which the new management team is actively auditing. The variety of activities available is mixed, with some residents feeling there is not enough to do, and a recommendation has been made to review and enhance the activity programme, particularly for those living with dementia.
Finally, in terms of being well-led, a complete change in the management team since the last inspection has impacted the consistent implementation of improvements. While new quality monitoring systems are being introduced, they are yet to be fully embedded to demonstrate sustained progress. There is an identified ongoing breach in governance and provider oversight, indicating a need for stronger leadership to drive consistent, high-quality care across all aspects of the service.
