What inspectors found

This inspection report for Moorland House highlights a service undergoing a significant period of transition following a change in ownership in July 2022. While residents and their families generally spoke warmly about the staff, felt welcomed when visiting, and noted that staff were mostly kind and approachable, the home currently faces serious challenges. A new management team, supported by a newly appointed manager and an external consultant, has an extensive improvement plan in place, particularly regarding the environment, but significant areas require immediate attention across all aspects of daily operation.

When looking at safety, numerous critical concerns were identified regarding risk management, infection control, and recruitment procedures. Important risk assessments and care plans were missing or outdated, medicines management showed multiple administrative gaps and stock discrepancies, and staff recruitment files lacked essential employment history and background checks.

Regarding effectiveness, care plans frequently contained conflicting information or lacked completeness, and staff did not consistently understand or apply the Mental Capacity Act frameworks. Furthermore, completion rates for specialist training were poor, leaving questions about staff competence, communication skills, and the proper support for residents with specific dietary needs.

On a positive note, residents were supported to make choices in their best interests, and staff maintained good working relationships with local healthcare professionals.

The caring aspect of the home was reflected in warm personal interactions, where longer-standing staff knew residents well and provided support without rushing. Relatives appreciated flexible visiting arrangements without the need for prior booking, and residents expressed trust in how their daily care and medication were handled.

However, responsiveness showed notable shortcomings, particularly in how incidents were investigated and how lessons were learned from accidents like falls. Care plans did not always adapt effectively to changing needs, such as ensuring call bells remained within residents' reach, and some staff struggled to respond appropriately due to communication barriers.

Finally, leadership and governance arrangements failed to identify significant safety and quality compromises prior to the inspection. Internal cultural issues among the staff team have hindered the new management's ability to drive improvements as quickly as desired, though the provider is actively taking steps to bring in extra support and address these concerns.
