Finally, regarding how well-led the service is, the inspection found that management and leadership were inconsistent. Audits were not always effective in identifying shortfalls, such as outdated medication resources or high agency staff usage not being flagged. There were also issues with the deployment of staff, with a resident requiring 24-hour support being left without their assigned staff member at times.
