This report summarises an inspection of The Orchards, a care home providing support for up to five individuals with learning disabilities. The inspection found that the service is well-regarded by residents, their families, and external professionals, who described staff as outstanding and noted positive relationships. The service aims to promote independence and choice in line with best practice guidance. While the report highlights many strengths, it also identifies areas for development.
Regarding safety, staff are well-trained in safeguarding and aware of how to protect residents from harm. Risks are assessed and managed, and appropriate emergency plans are in place. Recruitment processes are effective, ensuring suitable staff are employed, and there are sufficient staff to meet residents' needs, with one-to-one support provided when necessary. Medicine management systems are generally good, though improvements were noted regarding recording stock balances on medication records, and updates were planned for 'as required' medicine guidance. Infection control measures are in place, and the environment is clean. Incidents are recorded and reviewed to prevent recurrence.
In terms of how effective the care is, residents' needs are assessed before they move in, and support plans reflect individual preferences. Staff are skilled and knowledgeable, receiving good induction, training, and regular supervision. Residents are supported to maintain a healthy diet and are involved in meal preparation and shopping to promote independence. Staff work effectively with external professionals to ensure consistent care, and the environment is personalised, accessible, and comfortable. Residents are supported to access healthcare, and staff understand and apply the principles of the Mental Capacity Act, ensuring residents are involved in decisions where possible.
On caring, residents and their relatives express high satisfaction, describing staff as kind, compassionate, and dedicated. Staff have developed strong relationships with residents, understanding their individual needs and preferences. Residents are encouraged to express their views and are actively involved in decisions about their care, with regular meetings and feedback mechanisms in place. Suggestions from residents have led to tangible improvements, such as daily newspaper delivery. Staff treat residents with privacy, dignity, and respect, promoting independence and life skills.
Regarding how responsive the service is, staff have a deep understanding of residents' lives, preferences, and needs, with support plans regularly reviewed. Communication needs are considered, with staff adapting their approach to suit individual residents, and efforts are underway to improve communication support. Residents are supported to develop life goals, maintain relationships, and participate in a variety of community and culturally relevant activities, with examples including zoo visits and working at a go-karting club. Complaints are handled effectively and resolved to satisfaction, and end-of-life wishes are discussed and documented in detail.
Finally, on how well-led the service is, there is a clear oversight of operations and a good understanding of regulatory requirements, despite not having a registered manager currently in post. Internal and external audits are conducted, and any identified issues are addressed. The team leader actively participates in shifts, offering direct support and observing staff practice, fostering a culture of continuous improvement and person-centred care.
