What inspectors found

This report summarises an inspection of Foxburrow Grange, a residential and nursing care home. The inspection assessed the service against five key questions: Safe, Effective, Caring, Responsive, and Well-led. The overall findings indicate that while improvements have been made since the previous inspection, further attention is needed in several areas to ensure the highest quality of care.

On safety, while systems for safeguarding people and managing risks have improved, there are still areas requiring attention. This includes ensuring electrical sockets are assessed for tampering risks and that care records are consistently updated with critical information, such as dietary needs and choking risks, to ensure people are supported to eat and drink safely. While staffing levels have improved overall, the deployment of staff on the Hedgehog unit needs further review to consistently meet the changing needs of residents. Improvements have been made in medicine management, but attention is needed to ensure time-sensitive medicines are administered within the recommended timeframe.

In terms of how effective the care is, the service has demonstrated it is meeting some of the principles of supporting people with learning disabilities and autistic individuals, with staff training and helpful tools like social stories in place. They are also working well with other professionals to understand and meet people's needs, and promoting healthier lives, including a focus on oral hygiene.

Regarding how caring the service is, residents' privacy, dignity, and independence are respected. Staff are supported to care for people in the least restrictive way possible, and policies and systems are in place to uphold this practice.

In terms of how responsive the service is, while efforts are being made to improve end-of-life care through training and hospice collaboration, there are instances where staff response to pain management has not been quick enough. Care plans and risk assessments need further development to ensure they are current, reliable, and relevant, as repetitive information could lead to confusion.

Concerning how well-led the service is, while auditing processes have improved and identify areas for development, governance systems still need to encompass all quality and safety issues, including staffing and accuracy of care information. There is also work in progress to address concerns about the staff culture, focusing on improving morale, communication, and teamwork, with various methods in place to engage with residents, families, and staff.
