This report details an inspection of The Chilterns, a residential care home supporting individuals with mental health needs or learning disabilities. The inspection focused on assessing the safety, effectiveness, caring, responsiveness, and leadership of the service. While the home has made significant progress since its last inspection, particularly in improving staffing levels and addressing previous concerns regarding infection control and consent, several areas require ongoing attention to ensure consistently high-quality care for all residents.
Regarding the safety of the service, inspectors found that while improvements have been made in areas like infection control and staffing, there are still concerns. Care plans and risk assessments were not always updated following safeguarding incidents, and there were gaps in the information provided to staff, particularly agency staff, about how to best support residents. Medicines management also requires further attention, with inconsistencies noted in recording the application of medicated patches and a lack of clear guidance for applying medicated creams.
In terms of how effective the care is, the service has made progress in adapting its environment to meet residents' needs, with improvements in cleanliness and a plan for ongoing redecoration. Crucially, the service has demonstrated better practice in obtaining and documenting consent for care, moving away from blanket restrictions and ensuring residents have more choice and control over their lives.
When it comes to the caring aspect, feedback from residents indicated mixed experiences with staff, with some expressing discomfort with certain individuals. While safeguarding incidents are reported, the subsequent review and updating of care plans to reflect increased risks were not consistently thorough.
In terms of responsiveness, residents are now more involved in the service, with their opinions actively sought and acted upon, leading to more engaging activities. Restrictions that previously impacted residents' freedom have been reviewed and reduced, with individuals now having more autonomy to leave and return to the service.
Finally, regarding how well-led the service is, leadership and management have improved but remain inconsistent. While the manager is actively working on improvements and acknowledges areas needing further development, there are still breaches in good governance, meaning that systems for continuous learning and improving care are not always fully effective. The provider remains committed to addressing these areas.
